基于用药安全管理构建儿科门诊药房规范化培训体系

    Establish a Standardized Training in Pediatric Ambulatory Care Pharmacy Based on Medication Safety Management

      目的  基于用药安全管理,根据儿童用药风险点,构建儿科门诊药房规范化培训体系。
      方法 运用文献分析法构建门诊药师规范化培训体系基本框架,收集儿童门诊药品调剂风险因素,运用问卷调研法在试点儿童医院及其他妇儿医院门诊调研儿童门诊药品调剂风险因素,明确可通过规范化培训降低的风险因素,拟定儿科门诊规范化培训体系的模型,设计专家函询问卷。采用Delphi法进行2轮专家函询。
      结果 2轮问卷的回收率分别为100. 00% 及86.96%;2轮专家的权威系数分别为0.89、0.90;2轮评分的组内相关系数均>0.9。最终确立了4个层次的条目,其中1级条目5个、2级条目25个、3级条目43个,4级条目13个。
      结论 基于用药安全管理构建的儿科门诊药房规范化培训体系具有权威性、科学性、实用性及儿科药品调剂风险针对性,可降低药师人为因素对调剂安全的影响,保障儿童用药安全。

       

      Abstract:
      OBJECTIVE  To establish a standardized training in pediatric ambulatory care pharmacy according to the risk factors in pediatric patients based on medication safety management.
      METHODS  To analyze the literature to build the basic framework of standardized training in ambulatory care pharmacy, to collect risk factors in drug dispensing of pediatric ambulatory care, using a questionnaire survey to investigate the risk factors in drug dispensing of pediatric ambulatory care in pilot and other pediatric hospitals, to identify the risk factors that can be reduced through standardized training, to develop a model of standardized training in pediatric ambulatory care pharmacy, then design expert questionnaire, using Delphi method to conduct two rounds of expert consultation.
      RESULTS  Recovery rates of two rounds of questionnaires were 100.00% and 86.96%. Authority coefficients of two rounds were 0.89 and 0.90. The values of the intraclass correlation coefficient of two rounds were >0.9. The items of the standardized training had 4 levels with 5 in first level, 25 in second level, 43 in third level and 13 in fourth level.
      CONCLUSION The standardized training based on medication safety management in ambulatory care pharmacy for pediatric is authoritative, scientific, practical and targeted at the risk factors in drug dispensing for pediatric, it can reduce the impact of pharmacist factors on the risk of drug dispensing and ensure the safety of children’s medication.
