血府逐瘀汤治疗慢性阻塞性肺疾病的抗炎作用及对TLR4/MyD88/NF-κB信号通路的影响

    Anti-inflammatory Effect of Xuefu Zhuyu Tang in the Treatment of Chronic Obstructive Pulmonary Disease and Its Influence on the TLR4/MyD88/NF-κB Signaling Pathway

      目的 探讨血府逐瘀汤治疗慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)的抗炎作用及对TLR4/MyD88/NF-κB信号通路的影响。
      方法 前瞻性选取浙江中医药附属金华医院2021年1月—2023年1月收治的100例COPD患者,采用随机数表法分为观察组和对照组,各50例。对照组给予常规对症治疗,观察组在对照组基础上联合血府逐瘀汤。比较2组临床疗效,同时评估其中医证候积分,另外检测2组TLR/NF-κB信号通路表达情况以及炎性因子指标TNF-α、IL-1β、IL-6,最后观察记录不良反应的发生情况。
      结果 观察组总有效率(96.00%)显著高于对照组(84.00%),差异有统计学意义(P<0.05);2组治疗后咳嗽、喘息气急、失眠多梦、两目暗黑、舌紫暗、脉濡滑积分显著低于治疗前,且观察组显著低于对照组,差异均有统计学意义(P<0.05);2组治疗后TLR4 mRNA、MYD88 mRNA、NF-κB mRNA表达显著低于治疗前,且观察组显著低于对照组,差异均具有统计学意义(P<0.05);2组治疗后TNF-α、IL-1β、IL-6指标显著低于治疗前,且观察组显著低于对照组,差异均有统计学意义(P<0.05);2组不良反应总发生率比较差异无统计学意义。
      结论 血府逐瘀汤用于治疗COPD疗效显著,不仅能有效改善患者临床症状,还能改善其炎症反应,且安全性较高,其作用机制可能与调节患者TLR4/MyD88/NF-κB信号通路有关。

       

      Abstract:
      OBJECTIVE To explore the anti-inflammatory effect of Xuefu Zhuyu Tang in the treatment of chronic obstructive pulmonary disease (COPD) and its impact on the TLR4/MyD88/NF-κB signaling pathway.
      METHODS A prospective selection of 100 COPD patients admitted to Jinhua TCM Hospital Affiliated to Zhejiang Chinese Medical University from January 2021 to January 2023 was conducted. They were randomly divided into an observation group and a control group using a random number table method, with 50 cases in each group. The control group was given routine symptomatic treatment, and the observation group was combined with Xuefu Zhuyu Decoction on the basis of the control group. Compared the clinical efficacy of 2 groups, evaluated their traditional Chinese medicine syndrome scores, and tested the TLR/NF-κB signaling pathway expression and inflammatory factor indicators (TNF-α, IL-1β, IL-6), and finally observed and recorded the occurrence of adverse reactions.
      RESULTS The total effective rate of 96.00% in the observation group was significantly higher than 84.00% in the control group, with a statistically significant difference (P<0.05). After treatment, the scores of cough, wheezing and shortness of breath, insomnia and dreaminess, dark eyes, purple tongue, and smooth pulse in both groups were significantly lower than before treatment, and the observation group was significantly lower than the control group, with statistical significance (P<0.05). After treatment, TLR4 mRNA, MYD88 mRNA, NF-κB mRNA in both groups were significantly lower than before treatment, and the observation group was significantly lower than the control group, with statistical significance (P<0.05). After treatment, TNF-α, IL-1β, IL-6 index in both groups were significantly lower than before treatment, and the observation group was significantly lower than the control group, with statistical significance (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups.
      CONCLUSION  Xuefu Zhuyu Tang has a significant therapeutic effect on COPD. It can not only effectively improve clinical symptoms of patients, but also improve their inflammatory response, and has high safety. Its mechanism of action may be related to regulating the TLR4/MyD88/NF-κB signaling pathway in patients.
