    Development and Preliminary Evaluation of a PEAC Model-based Pharmaceutical Care Service Model for Perioperative VTE Management in Orthopedics

    • Pharmaceutical service for the prevention and treatment of venous thromboembolism(VTE) in the perioperative period of orthopedics is a novel pharmaceutical service model developed based on pharmaceutical active consultation. It provides differentiated and proactive pharmaceutical care for orthopedic patients assessed as having medium to high risk of VTE during the perioperative period, serving as one of the specialized services within PEAC. Traditional VTE prevention and management models often establish unified prevention and treatment protocols, managing VTE based on the risk assessment results of specific patients. In contrast, the differentiated pharmaceutical care for VTE in orthopedic perioperative patients fully considers the distinct VTE risk factors and clinical circumstances of orthopedic surgical patients compared to other surgical and non-surgical patients. It establishes VTE prevention and treatment protocols and pharmaceutical service models that differ from general VTE management processes, better aligning with the actual needs of orthopedic surgical patients. In this pharmaceutical service, clinical pharmacists proactively screen patients at risk of VTE to provide timely and comprehensive pharmaceutical care. Simultaneously, they act as communication bridges within the multidisciplinary VTE prevention and treatment team, enhancing collaboration among multidisciplinary teams. This offers new references and insights for medical institutions to explore efficient pharmaceutical service models for VTE prevention and treatment.
