      目的 基于uni-app设计并开发慢病管理系统,解决患者院外健康监测中断、自我管理不足的问题。
      方法 结合药学与计算机科学,利用移动通信及语音智能互动技术,开发“掌上医管家”慢病管理系统,并详细论述该系统中提醒、监测、咨询、科普四大模块的功能与应用。
      结果 该系统通过多学科团队为患者量身定制简单易用、个性化的院外健康管理计划,以图文声并茂的方式推送至患者,促使患者按时执行慢病管理计划,并通过主动监护收集完整的监测数据,为临床动态调整诊疗方案提供依据。经测试,该系统运行稳定、流畅,可操作性强,通俗易懂,简单易用。
      结论 通过信息化建设赋能,该系统有望提高慢病的防控效能,为实现全程化、智能化、适老化的现代慢病共管服务体系提供支持。

       

      Abstract:
      OBJECTIVE To design and develop a chronic disease management system based on uni-app, so as to address the problems of interrupted off-hospital health monitoring and insufficient self-management among patients.
      METHODS A system named “Palm Medical Management” for chronic disease management was developed by integrating pharmacy and computer science with mobile communication and voice intelligent interaction technologies. The functions and applications of its four core modules were discussed in detail, including reminders, monitoring, consultation, and health science popularization.
      RESULTS The system customized easy-to-use and personalized out-of-hospital health management plans for patients through a multidisciplinary team, and delivered them to patients in a graphic-text-voice manner. This facilitated the timely implementation of chronic disease management plans by patients. Meanwhile, complete monitoring data were collected through active health surveillance, which laid a foundation for clinicians to dynamically adjust diagnosis and treatment regimens. Tests proved that the system operated stably and smoothly with simple operation and a user-friendly interface.
      CONCLUSION Empowered by information construction, this system is expected to improve the prevention and control of chronic diseases, and provide support for the realization of a modern chronic disease co-management service system that is full-fledged, intelligent and age-appropriate.
