基于IL-6/STAT3通路的大黄蛰虫丸调节Th17/Treg平衡治疗血瘀证子宫内膜异位症的机制

    Mechanism of Dahuang Zhechong Pill in Regulating Th17/Treg Balance for Endometriosis with Blood Stasis Syndrome Based on the IL-6/STAT3 Pathway

      目的 从IL-6/STAT3通路调控异位子宫内膜组织Th17/Treg平衡,探讨大黄蛰虫丸治疗子宫内膜异位症(endometriosis,EMs)血瘀证的潜在分子机制。
      方法 60只雌性SD大鼠按照随机数字表法分为假手术组、模型对照组、阳性对照组(地诺孕素)及大黄蛰虫丸高、中、低剂量组,每组10只。采用自体移植复合注射盐酸肾上腺素的方法建立血瘀证EMs大鼠模型。给药14 d后观察各组大鼠一般状况,然后处死大鼠,测量异位组织体积,测定血常规、血液流变学及凝血四项指标凝血酶原时间(prothrombin time,PT)、活化部分凝血酶原时间(activated partial thromboplatin time,APTT)、凝血酶时间(thrombin time,TT)和纤维蛋白原(fibrinogen,FIB),观察异位组织病理变化,检测血清性激素、炎症因子水平、异位组织p-STAT3蛋白表达,以及IL-6、STAT3、RORγt、Foxp3蛋白和mRNA的表达。
      结果  与假手术组比较,模型对照组大鼠舌色暗紫,毛发不整,异位组织体积显著增大,组织病理学结果符合异位子宫内膜组织病理学特征;中性粒细胞/淋巴细胞比值(neutrophil lymphocyte ratio,NLR)、系统性免疫性炎症指数(systemic immune inflammation index,SII)均显著升高(P<0.01);全血黏度及血浆黏度值均显著升高(P<0.01);PT、APTT、TT均显著降低(P<0.01),FIB值显著升高(P<0.01);血清E2、TNF-α、IL-17及IL-6含量均显著升高(P<0.01),而IL-10、TGF-β1含量均显著降低(P<0.01);异位组织p-STAT3蛋白表达,IL-6、RORγt蛋白与mRNA表达,以及STAT3 mRNA表达均显著升高(P<0.01),而Foxp3蛋白与mRNA表达显著降低(P<0.01)。与模型对照组比较,阳性对照组及大黄蛰虫丸高、中剂量组大鼠状态明显改善,异位内膜组织体积显著减小(P<0.01),病理损伤不同程度改善;NLR、SII 值显著降低(P<0.01);全血黏度及血浆黏度值均显著降低(P<0.01);PT、APTT及TT值显著升高(P<0.01), FIB值显著降低(P<0.01);血清E2、TNF-α、IL-17、IL-6含量均显著降低(P<0.01),而IL-10、TGF-β1含量均显著升高(P<0.01);异位组织p-STAT3蛋白表达,IL-6、RORγt蛋白与mRNA表达,以及STAT3 mRNA表达均显著降低(P<0.05或P<0.01),而Foxp3蛋白及mRNA(除阳性对照组外)表达显著升高(P<0.05或P<0.01)。
      结论 大黄蛰虫丸可能通过干预IL-6/STAT3信号通路关键蛋白,调控异位子宫内膜组织Th17/Treg平衡,对血瘀证EMs起到治疗作用。

       

      Abstract:
      OBJECTIVE To explore the potential molecular mechanism of Dahuang Zhechong Pill in treating endometriosis(EMs) with blood stasis syndrome by regulating Th17/Treg balance in ectopic endometrial tissue through the IL-6/STAT3 pathway.
      METHODS Sixty female SD rats were randomly divided into a sham operation group, a model control group, a positive control group(dienogest), and high, medium, and low dose groups of Dahuang Zhechong Pill according to a random number table method, with 10 rats in each group. A blood stasis syndrome EMs rat model was established using autologous transplantation combined with injection of adrenaline hydrochloride. After 14 d of administration, the general condition of each group of rats was observed, and then the rats were euthanized. The volume of ectopic endometrial tissue was measured, and the blood routine parameters, hemorheological indices, and four coagulation testsprothrombin time(PT), activated partial thromboplatintime(APTT), thrombin time(TT) and fibrinogen(FIB) were determined. Pathological changes in ectopic endometrial tissue were observed, and serum levels of sex hormones, inflammatory factors, p-STAT3 protein expression, IL-6, STAT3, RORγt, and Foxp3 protein and mRNA expression in ectopic endometrial tissue were detected.
      RESULTS Compared with the sham operation group, the model control group exhibited dark purple tongue color and disheveled fur, showed significantly enlarged ectopic endometrial tissue volume, and had histopathological findings consistent with the characteristics of ectopic endometrial tissue; both neutrophil lymphocyteratio(NLR) and systemic immune inflammation index(SII) values were significantly elevated(P<0.01); its whole blood viscosity and plasma viscosity were significantly increased(P<0.01); the PT, APTT, and TT values were significantly decreased(P<0.01), while the FIB value was significantly increased(P<0.01); serum E2, TNF-α, IL-17, and IL-6 levels were significantly increased(P<0.01), while IL-10 and TGF-β1 levels were significantly decreased(P<0.01). The expression of p-STAT3 protein, IL-6 and RORγt protein and mRNA, and STAT3 mRNA in ectopic endometrial tissue were significantly increased(P<0.01), whereas Foxp3 protein and mRNA expression were significantly decreased(P<0.01). Compared with the model control group, the positive control group and the high- and medium-dose Dahuang Zhechong Pill groups showed significant improvement in their condition, with a significant reduction in the volume of ectopic endometrial tissue (P<0.01) and varying degrees of improvement in pathological damage; the NLR and SII values significantly decreased(P<0.01); whole blood viscosity and plasma viscosity were significantly decreased(P<0.01); the PT, APTT, and TT values significantly increased(P<0.01), while the FIB value significantly decreased(P<0.01); serum E2, TNF-α, IL-17, and IL-6 levels were significantly decreased(P<0.01), while IL-10 and TGF-β1 levels were significantly increased(P<0.01); the expression of p-STAT3 protein, IL-6 and RORγt protein and mRNA, and STAT3 mRNA in ectopic endometrial tissue were significantly reduced(P<0.05 or P<0.01), whereas Foxp3 protein and mRNA expression (except for the positive control group) were significantly increased(P<0.05 or P<0.01).
      CONCLUSION Dahuang Zhechong Pill may have a therapeutic effect on endometriosis with blood stasis syndrome by intervening in key proteins of the IL-6/STAT3 signaling pathway, regulating Th17/Treg balance in ectopic endometrial tissue.
