Importance of Pharmaceutical Care for Elderly Discharged Patients with Low Cultural Level

OBJECTIVE To explore the influence of medication guidance from clinical pharmacist on safety and compliance of dischanged patients with low cultural level. METHODS The discharged patients above 60 years old without primary school education received a special medication guidance. Clinical pharmacists designed medication cards for reminding them of administration. RESULTS & Conclusion Base on the elderly discharged patients’ compliance, maintenance and drug knowledge, the medication cards designed by clinical pharmacists can help to improve effectiveness and safety of disease control.
