    Effect of Two-way Referral Mode Participated by Clinical Pharmacists on Systemic Lupus Erythematosus Patients

    • OBJECTIVE To explore the effect of the two-way referral mode of tertiary cooperated with community hospitals participated by clinical pharmacists on the clinical outcome of systemic lupus erythematosus(SLE) patients. METHODS Patients with SLE in Renji Hospital, School of Medicine, Shanghai Jiao Tong University were intervened by the two-way referral mode of tertiary cooperated with community hospitals participated by clinical pharmacists. The main outcomes were the score of SLE disease activity index(SLEDAI-2K) and EuroQol five-dimension(EQ5D) questionnaire at baseline and 12 months. RESULTS From June 2016 to November 2018, 82 patients with SLE were intervened by the treatment mode. At 12 months, the result of SLEDAI-2K was significantly improved compared with baseline(7.72 vs 2.05, P=0.000), and quality of life evaluated by EQ5D(0.733 vs 0.825, P=0.010) were also improved. CONCLUSION The two-way referral mode of tertiary cooperated with community hospitals participated by clinical pharmacists proved to be an effective auxiliary clinical treatment mode, can effectively improve the clinical outcome and quality of life of patients.
