Discussion on the Value of Clinical Pharmacists in the Management Team of Concomitant Chronic Diseases in County

OBJECTIVE Under the background of "medical community" construction and hierarchical diagnosis and treatment model, the contract model of "family doctor" was used for reference to explore the management team of doctor, medicine and care of chronic diseases involving pharmacists. And analyzed the role of pharmacists in the detailed management of chronic diseases among grassroots patients. METHODS To analyze the value of clinical pharmacists' participation in medication management of chronic disease management patients. RESULTS The intervention of clinical pharmacists was an essential part in the rational diagnosis and treatment of chronic diseases. CONCLUSION In order to ensure the continuous development of the management team, it is imperative to improve the supporting facilities.
