Investigation and Case Analysis of Typical Clinical Pharmacists Participating in Consultation of Elderly Patients in Cardiology

OBJECTIVE To investigate the consultation of clinical pharmacists in the Second People's Hospital of Jiaozuo City for elderly patients with cardiology and share typical cases, and to provide reference for further improving clinical pharmacy services. METHODS To analyze the drug use and typical cases of elderly patients with cardiology in the clinical pharmacist consultation or active monitoring from January 2017 to June 2018 in the Second People's Hospital of Jiaozuo City. RESULTS Clinical pharmacists participated in 57 cases of consultation or active monitoring of elderly patients in cardiology. The common diseases were hypertension, arrhythmia, coronary heart disease, etc. The common drugs for consultation were antihypertensive drugs, antibacterial drugs, antiarrhythmic drugs, etc. The consultation rate was 78.95%(completely adopted 43.86%, partially adopted 35.09%), the effective rate of consultation was 75.44%. CONCLUSION Clinical pharmacists should actively participate in clinical practice, form a treatment team with physicians, timely discover and solve problems in medication, and promote clinical rational drug use.
