| 目的分析静脉药物配置中心的不合理处方，确保患者用药安全。方法临床药师审查静脉药物配置中心医嘱，根据临床药理学知识及文献分析不合理情况。结果共审查配置中心输液卡155 028张，发现不合理816张，主要包括溶媒选择不合适、药物浓度不合适、配伍禁忌、给药间隔不当、医嘱录入错误等方面。通过采取干预措施后，不合理医嘱出现率由0.53%下降到0.17%。结论每日审核静脉药物配置中心处方可减少不合理医嘱，提高患者静脉用药的安全性。 |

| Abstract: |
| OBJECTIVE To analyze the unreasonable prescriptions in pharmacy intravenous admixture services(PIVAS) to ensure patients medication safety. METHODS Clinical pharmacist examined the prescriptions in PIVAS and analyzed the unreasonable prescriptions according to pharmacological knowledge and literatures. RESULTS A total of 155 028 prescriptions was examined, and 816 pieces of unreasonable prescriptions were found, involving problems in improper choice of infusion carriers, unsuitable concentration, incompatibility, improper interval of drug administration and erroneous advising. With the intervention measures method, the rate of irrational prescriptions were reduced from 0.53% to 0.17%. CONCLUSION Examining prescriptions in PIVAS can decrease the irrational administration and improve the safety of patients use of intravenous drugs by the daily work. |
