1例阿加糖酶α相关的心脏受累患者心室电风暴病例分析

    A Case Analysis of Ventricular Electrical Storm in a Patient with Cardiac Involvement Associated with Agalsidase Alfa

    • 摘要: 目的 分析1例法布雷病患者使用阿加糖酶α后出现心室电风暴的病例,探讨不良反应关联性及临床应对策略,为保障用药安全提供参考。方法 临床药师结合药物时序关联及排除其他诱因,判定该不良反应与阿加糖酶α的关联性为“可能相关”,并协助制定停用阿加糖酶α、给予胺碘酮及电复律等综合治疗方案。结果 患者心率失常逐渐控制,病情稳定;分析提示阿加糖酶α可能通过心肌酶代谢扰动或直接心肌刺激诱发电风暴。结论 阿加糖酶α致心脏毒性虽罕见但可能危及生命,临床对法布雷病患者行酶替代疗法时需严格评估心脏基础状态,制定个体化给药方案并加强心电监测,提高不良反应识别与防控意识。

       

      Abstract: OBJECTIVE To analyze a case of ventricular electrical storm following agalsidase alfa administration in a patient with Fabry disease, explore the causality assessment of the adverse reaction and clinical management strategies, and provide reference for ensuring medication safety. METHODS Clinical pharmacists, based on the temporal relationship of drug administration and exclusion of other triggers, assessed the causality between the adverse reaction and agalsidase alfa as "possible," and assisted in formulating a comprehensive treatment plan including discontinuation of agalsidase alfa, administration of amiodarone, and cardioversion. RESULTS The patient's arrhythmia was gradually controlled and the condition remained stable. Analysis suggested that agalsidase alfa may induce electrical storm through disturbance of myocardial enzyme metabolism or direct myocardial stimulation. CONCLUSION Although cardiotoxicity induced by agalsidase alfa is rare, it may be life-threatening. Clinicians should strictly evaluate the baseline cardiac status of Fabry disease patients before initiating enzyme replacement therapy, formulate individualized dosing regimens, strengthen electrocardiographic monitoring, and enhance awareness of adverse reaction recognition and prevention to ensure medication safety.
